What is Faecal Incontinence?

Faecal incontinence is an inconvenience characterised by the unexpected and involuntary leakage of faeces. A sudden bout of diarrhoea, soft stools, or damage to the rectal and anal muscles can impact your ability to control your bowel movements, resulting in faecal incontinence.

Although this condition is common to both genders, it is more common in women. However, aged men can also experience faecal incontinence due to various problems. According to experts, approximately 1 in 12 adults suffer from faecal incontinence. Despite its prevalence, many avoid seeking help due to stigma. 1

At JOGO Health, the leading digital therapeutic clinic in India, we help you overcome this inconvenience with our proven, painless, and non-invasive treatment methods. Our experts guide you with exercises to strengthen your pelvic, rectal, and anal muscles to regain bowel movement control.

Causes of Faecal Incontinence?

Faecal incontinence occurs when you lose control of the rectal muscles. Here’s a list of medical conditions and other factors that cause this inconvenience.
Diarrhoea
The rectal muscles have a harder time retaining loose, watery stools compared to firm ones, which can lead to involuntary faecal voiding.
Constipation
The inability to defecate due to frequent bouts of constipation can weaken the rectal and anal muscles. This condition can lead to seepage of the liquid parts of the stool.
Muscle & Nerve Damage
Complicated vaginal delivery, use of forceps to facilitate childbirth and episiotomies can damage the anal muscles leading to faecal incontinence in women. The same procedures can also adversely impact the nerves controlling the movement of these muscles.
Lack of Rectal Elasticity
The rectal muscles play an essential role in holding faeces until it is time to defecate. These muscles need to be sufficiently flexible to prevent stool leakage. If the muscles lack elasticity, it can result in faecal incontinence.
Pelvic Organ Prolapse
When the pelvic muscles become weakened and fail to provide adequate support to the pelvic organs, it is known as Pelvic Organ Prolapse (POP). A rectal prolapse, on the other hand, occurs when the rectum collapses into the anus, which may result in faecal incontinence.
Medical Conditions
Faecal incontinence is a common occurrence in individuals suffering from nerve-related conditions like Dementia, Parkinson’s disease, Alzheimer’s Diabetes, Multiple Sclerosis, etc. These chronic conditions can weaken the nerves and muscles, resulting in faecal incontinence.

Complications Caused by Faecal Incontinence

Unless treated on time, faecal incontinence can cause several complications that extend beyond physical issues. It is advisable to seek help from experts before this condition compromises your physical and mental health. The complications caused by faecal incontinence include:
Emotional & Social Distress
Generally, faecal incontinence is considered a major embarrassment. Individuals may avoid socialising with people. They may become aloof and depressed.
Physical Inconveniences
The frequent need to defecate and clean oneself can irritate the sensitive skin around the anus, leading to itching, burning sensation, and pain. These issues may eventually develop into painful ulcers in that area.
Nutritional Deficiencies
Chronic faecal incontinence signals that your body is being depleted of nutrition, for which you may require nutritional supplements.

Treatments For Faecal Incontinence

Faecal incontinence can be easily rectified and managed, provided the causative factors are accurately diagnosed. Depending on the causes, you may require a combination of treatments and lifestyle modifications to manage your issue more effectively. It includes:

JOGO’s Biofeedback Therapy

Our biofeedback therapy is an entirely non-invasive treatment that helps you control your rectal muscles, which are vital for bowel control. (EMG) Electromyographic biofeedback therapy for faecal incontinence uses a sensor, computer, video monitor and small electric probes. The probes are placed at your bottom, and the electric signals are transmitted to the computer.

The relayed information allows our experts to understand the pressure and movement of the muscles in your rectum. They will ask you to perform a customised series of exercises to improve your bowel control. The sensor verifies that you are correctly performing the exercises.

Dietary Modifications

Your food choices could be the underlying cause of certain faecal incontinence issues, such as constipation and diarrhoea. You can prevent and regain control over your bowel movements by avoiding certain foods, modifying your consumption levels, and tracking what you eat or drink.

If you are experiencing diarrhoea, avoiding foods that can cause loose stools or the urge to go is important. The list includes greasy and fatty foods, fibre-rich fruits and vegetables, beans and cabbage, caffeinated products, prunes and certain fruit juices, cured or smoked meat, spicy foods, dairy products, and alcohol. However, if chronic constipation is your problem, your dietary modifications should include fibre-rich foods like green leafy vegetables, whole grains, and nuts. Good water intake of at least 8-10 glasses per day is also recommended.

Physical Activity

Regular exercise and training are deeply connected with your bowel control. Therapists recommend Kegel exercises to strengthen your pelvic muscles to regain the lost control over your bowel movements. These muscles are vital in supporting your bladder and rectum. Kegel exercises are simple workouts that alternate between squeezing and relaxing your muscles.

Doing this activity regularly will help strengthen these muscles and regularise your bowel movement pattern. Our JOGO therapists will help you gain the maximum benefits from these exercises.

Medications

Medications like bulk laxatives, enemas, and suppositories are prescribed to help you overcome chronic constipation. Specialist doctors may also prescribe anti-diarrhoeal drugs to counteract diarrhoea caused by bacterial infections or wrong food intake.

You must avoid resorting to over-the-counter medications without your doctor’s guidance. If over-the-counter medications prove ineffective, a prescription may be necessary to manage faecal incontinence.

Frequently Asked Questions On Treatment For Faecal Incontinence

What are the types of faecal incontinence?

Faecal incontinence is of two types:

  • Urge incontinence is when you feel the urge to defecate, but you are unable to control the urge until you make it to the restroom and end up soiling yourself. Urge incontinence is prevalent. Generally, it pertains to problems with the muscles responsible for regulating your bowel movements.
  • Passive incontinence is an involuntary bowel movement that occurs when the rectum is stretched to its maximum holding capacity, but the body fails to recognise the need to defecate. It occurs when there’s a breakdown in the communication between the nerves and muscles.

How is faecal incontinence diagnosed?

Apart from a physical examination, your doctor may recommend anorectal manometry - a diagnostic test to measure pressures in the anus and rectum and to assess their function. The results of this test will shed light on the causes of your incontinence issue.

What measures can I take to prevent faecal incontinence?

Although most causative factors for faecal incontinence are unpreventable, the correct diagnosis can lead you to the most suitable solution for this problem. For instance, if your diet triggers your incontinence, you can try avoiding certain foods like high-fibre vegetables, caffeinated drinks, alcohol, etc. You may also modify your bowel habits with proper bowel training and develop a bathroom schedule to prevent incontinence accidents.

Will faecal incontinence last for years or months?

The duration of this issue generally depends on the causative factors. For instance, if you experience faecal incontinence due to dysentery or diarrhoea, a dosage of antibiotics is more than enough to kill the bacterial infection causing it. The matter can be resolved in a day or two.

Therapies such as biofeedback can help improve bowel control, but their effectiveness may vary depending on the patient's condition prior to treatment.
